3.   The amount of the mortgage recorded in Liber/ Reel ________ at Page _______ is $25,000.00 or less and which mortgage has matured over 12 years.

4.   No demand for payment has been made by the mortgagee or assignees. 

5    I make this statement knowing full well that ___________________, as agents for Stewart Title Insurance Company relies on the accuracy thereof for the purposes of issuing its policy of title insurance free and clear of the aforesaid.
